Dashboard

Go to Dashboard and select the course. To create a new feedback for a course,click 'Feedback' tab in the course details page and click 'Create Feedback'.

Evaluation Form

Evaluation form is loaded with a few sample questions under three sections. You can modify or delete the sample questions. Enter a name for the feedback and title(will be seen by delegate). Select whether the form is for delegates/faculty. Also check if the form is optional or madatory for delegates/faculty. Selecting optional will allow the delgates to skip the form and download the certificates directly. 

Questions can be added/edited in the existing sections or new sections can be added.

Add Section

Click 'Add section' to create a new section. 

Select any of the predefined heading or select custom heading and add subheading title to add a new section name. When the delegate fills the form each section is shown in a different page.

Add Question

Click 'Add Question' to create a new question. 

Enable 'Required' if this question is a mandatory one. Enter question and select the type of question. Enter answer choices like in the image for the type Dropdown,Checkboxes, Radios and Grid Radio. 

If any of the section has to be skipped, 'required' checkbox in all the questions below  a section should not be selected. This will allow the delegate to skip the section which he/she has not attended.

Grid Radio

Grid radio question type is best used when you want to get rating for different aspects of a lecture or workshop. For example when you want to compare 'Presentation' and 'Content' or a lecture use grid radio rather than two different questions under radio type. Upto 3 aspects can be compared.

Copy Question

Similar questions can be copied  within the section using copy question. The copied question is placed next to the question which is copied. This make it easier and quicker to create a form. If you have 10 lectures, just create the first question for first lecture and then copy it over and change the text of the question.

Rearranging questions

Questions can be reordered by drag and drop., after reordering click 'Save Feedback Form' to persist the changes.This can be done only within the section.

Save Feedback form

Click 'Save Feedback Form' to save the current changes and this can be edited/deleted/preview later from the feedback dashboard page like in the image.

Publish - Click 'Publish' when the feedback form is fully ready. You can only send 'published' feedback form to delegates or faculty. See our how to page on how to send the feedback form.
